To complete the membership application, do either of the following:

1.  Print out the form, fill it out, fax it to the fax number at the top of the form.

2.  Print out the form, fill it out, mail it to the address on the form.

3.  Print out the form, fill it out, drop it by the Pro Shop.

4.  Copy the form to a text editor.  Fill it out.  Save it as a text file to your computer.  Send an email with the file attached to debra@pontotoccc.com
